SOUTHINGTON, CT — Southington police Wednesday released a report on a fatal crash over the Memorial Day weekend.

According to a crash report, at 7:06 Sunday morning, 78-year-old John Walsh was traveling west on Columbus Avenue in a blue 2002 Ford Explorer.

He was the sole occupant of the vehicle, police said.

The Ford suddenly shifted to the eastbound lane and left the south side of the road, according to a crash report. It then hit a fence along the southbound side of Columbus Avenue, continued through the intersection of Columbus Avenue and Liberty Street and struck the front of 2 West Center St., according to a crash report.

Despite lifesaving efforts on the part of an ambulance crew, Walsh died at the scene, according to a crash report.
