GWINNETT COUNTY, GA — Here are some restaurant and food service inspections conducted in Paulding County during the past week. Click on the date for more info.

Each item on an inspection form has a point value ranging from 1 to 9 points, with violations deducting points from a best possible score of 100. Higher points are taken for items with higher risk to cause illness, while repeat violations take even more points.

Letter grades assigned are A for totals of 100 to 90 points, B for 89 to 80, C for 79 to 70, and U for less than 70. Restaurants graded C and lower have their violations listed.

1-2A: .03(2)(a)-(o)Multiple violations occurred on todays inspection and many of which were repeats from the previous inspection on 3/12/2020 which displays a lack of managerial control It is the responsibility of the person in charge to ensure compliance with the food code by demonstrating active managerial control within their facility.
Corrective Actions: Some violations were corrected on site, while others could not be correct on site.

2-1B: .03(5)(c)Two employees touching their face, hair, adjusting their clothes without appropriate handwash on food prep line/drive thru Employees must wash hands and exposed portions of arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service articles, and: 1) after touching exposed body parts; 2) after using the restroom; 3) after coughing, sneezing, using tobacco, eating, or drinking; 4) after handling soiled equipment or utensils; 5) during food preparation, as often as needed to remove soil and contamination and to prevent cross contamination when changing tasks; 6) before putting on gloves; 7) after engaging in other activities that contaminate hands; 8) a second time when entering the kitchen from using the restroom.
Corrective Actions: **Inspector spoke on appropriate hand washing and PIC in training corrected accordingly.

2-2A: .03(2)(m)-Employee Health Policy Agreements could not be located at time of inspection Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food.
Corrective Actions: Could not correct on site.

4-2B: .05(7)(a)1-Pans/trays in the walk-in cooler with food debris, fryer with debris, drive-thru drink machine with mold-like substance and debris, interior of reach in coolers with debris, self-serve drink machine nozzles in dining area with debris and mold-like substance/ ice chute with mold like substance, self-serve iced tea dispenser nozzle with debris Food contact surfaces must be maintained clean and sanitized to prevent cross contamination, growth of bacteria/mold, etc.
Corrective Actions: Could not correct on site.

5-1B: .04(5)(h)Frozen corn not meeting 2 hr threshold for 165 degree requirement. temped out at 128-133 Time/temperature control for safety food that is cooked, cooled, and reheated for hot holding shall be reheated so that all parts of the food reach a minimum temperature of 165°F (74°C) within 2 hours.
Corrective Actions: Corn was discarded.

6-1B: .04(6)(f)Fries and the rice and beans on hot hold line were out of temperature parameter of 135 degrees or higher. Temps. observed were 122-133 degree Time/temperature control for safety food shall be maintained at 135°F (57°C) or above.
Corrective Actions: Fries and rice and beans were discarded.

11C: .04(6)(c)Raw frozen chicken left at room temperature to thaw across from three compartment sink Time/temperature control for safety shall be thawed: 1) under refrigeration that keeps the food at 41°F or below; 2) completely submerged in running water with the temperature of the water at 70°F or below, and the no part of the food exceeding 41°F; 3) as a part of the cooking process.
Corrective Actions: Raw chicken was moved to walk in cooler to thaw.

12B: .03(5)(f)-Employee with long, artifical nails engaging in food service Unless wearing gloves in good repair, a food employee may not wear fingernail polish or artificial fingernails when working with exposed food. Food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food (except for a plain ring such as a wedding band).
Corrective Actions: Employee was stopped engaging in food prep.

13A: .02(1)(d)No inspection report displayed 1.The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. 2.Food service establishments with drive-thru windows will post the current inspection report, and also have the inspection report posted so that a minimum of the top one-third of a copy of the current inspection report is visible through each window allowing customers to easily read the score, date of inspection and establishment information.
Corrective Actions: Today's inspection report will be displayed.

14B: .05(10)(e) 1,2,4Pans stacked on top of each other not allowing for adequate air drying "After cleaning and sanitizing, equipment and utensils: 1. Shall be air-dried or used after adequate draining before contact with food; and 2. May not be cloth dried except that utensils that have been air-dried may be polished with cloths that are maintained clean and dry."
Corrective Actions: Could not correct on site.

15A: .05(1)(i)-Walk-in cooler and freezer shelves, reach-in cooler shelves, doors, door handles, exterior of fryers, exterior of single service items, exterior of hot hold containers on prep line, exterior of drink machines, condiment containers, exterior of hot boxes-- all soiled with debris and or stripping/galvanizing All non- food contact surfaces must be maintained clean, sanitized, non-absorbant and in good repair
Corrective Actions: Could not correct on site.

16A: .06(1)(g),(h)Hot water at men's bathroom sink in the establishment did not exceed 72-78 degrees Fahrenheit after letting the water run extensively. Hot water of 100 degrees minimum must be available at all times at all sinks.**Inspector educated that hot water must be available at all times in order or properly handwash, clean and sanitize thus the lack thereof can promote lack of safety/cleanliness. Hot water should be reinstated ASAP
Corrective Actions: Could not correct on site.

17B: .06(5)(r),(s)Debris on and around the garbage dumpsters on the exterior of the building in the parking lot. Waste shall not be allowed to build up in the receptacle area nor be placed on the ground in order to prevent environmental contamination and presence of vermin (e.g. rats, flies, etc.)**Inspector said that cleaning needs to be done

17C: .07(5)(a),(b)-Floors, walls and ceilings covered in debris, stains, standing water, and/or in disrepair throughout the establishment. Premises must be maintain clean, sanitized and in good repair **Inspector advised that the premises needs to be deep cleaned and sanitized; and showed PIC in training everywhere that needs to be attended to
Corrective Actions: Could not correct on site.

18: .07(5)(k)-Several Flies observed in drive-thru area , batter station and in dumpster area - 2 roaches in the underside of service counter by front area Vermin are not permitted on premise and active measures need to be taken to prevent their presence **Advised that pest control be called and the premise be cleaned and sanitized

18: .07(2)(m)1. Drive thru window was left open. 2. Back door was propped open. "Outer openings of a food service establishment shall be protected against the entry of insects and rodents by: 1) filling or closing holes and other gaps along floors, walls, and ceilings; 2) closed, tight-fitting windows; and 3) solid self-closing, tight-fitting doors."
Corrective Actions: 1. Drive thru window was closed. 2. Back door was closed.

Person in charge shall explain their responsibility for preventing the transmission of foodborne disease by a food employee who has a disease or medical condition that may cause foodborne disease and can describe the symptoms associated with the diseases that are transmissble through food. They also shall be able to explain how food employees and conditional employees comply with reporting responsibilities and exclusion or restriction of food employees.
Corrective Actions: The employee health policy was emailed to person in charge and reviewed during inspection.

2-2A: .03(2)(m)Person in charge could not provide the signed employee health policy agreements. The employee health policy agreement was emailed to person in charge during inspection. Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food.

2-2B: .03(5)(j)1&2Observed an open employee Pepsi bottle stored on the prep counter with single use pizza boxes stored below employee drink. Employees shall consume food and drink only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas (unless drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee's hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles).
Corrective Actions: Employee drink was discarded.

2-2E: .03(6)Person in charge could not provide facility's vomit and diarrheal clean-up kit. Facility must have clean-up procedures, clean-up supplies, and a sanitizer that is effective against Norovirus on site by 3/27/2020. A food establishment shall have procedures for employees to follow when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. The procedures shall address the specific actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food, and surfaces to vomitus or fecal matter.

6-1D: .04(6)(i)Per person in charge, the garlic sauce, garlic parmesan sauce, ranch sauce, and alfredo sauce are stored on a 4 hour time control. Foods were not properly marked with a start or discard time. Per person in charge, foods had been on time control for less than 4 hours. Person in charge could not provide facility's time control procedures. "When using Time Control (TPHC) for a maximum of 4 hours: 1) hot foods shall start at 135°F (57°C) or above and cold foods shall start at 41°F (5°C) or below; 2) the food items shall be labeled with a start and discard time, not to exceed 4 hours; 3) foods not consumed before the discard time shall be discarded; 4) when combining ingredients, the start/discard time for the foods shall reflect the start/discard time of the earliest ingredient placed on time control; 5) written procedures TPHC shall be created in advance and made available upon the Health Authorities request."
Corrective Actions: Foods were marked with the start and discard time. Time control procedures emailed to person in charge during inspection.

8-2B: .07(6)(c)At time of inspection, observed several chemicals stored on and above customer food and single use items. Poisonous or toxic materials shall be stored and displayed for retail sale so they cannot contaminate food, equipment, utensils, linens, and single-service and single-use articles.
Corrective Actions: Chemicals were relocated.

9-2: .10(5)(c)2 containers of pizza sauce and individual containers of pizza sauce were not marked with the start and discard times. Person in charge could not provide the State approved variance for the pizza sauce during inspection. "If granted a variance or a HACCP plan is otherwise required, the permit holder shall: 1) Comply with the HACCP plans and procedures that are submitted and deemed in conformance with DPH Rule 511-6-1-.02(6)(a) through (e) as a basis for the modification or waiver; and 2) Maintain and provide to the Department, upon request, records specified under DPH Rule 511-6-1-.02(6)." **4th CONSECUTIVE REPEAT VIOLATION ON A ROUTINE INSPECTION**
Corrective Actions: 2 containers of pizza sauce were marked with the start and discard time. Person in charge did not know how long the individual pizza sauce containers had been stored at room temperature. Individual pizza sauce containers were discarded. The State approved variance for the pizza sauce was emailed to the person in charge during inspection.

12A: .04(4)(q)At time of inspection, observed food items stored in a plastic container that was in direct contact with the floor (food stored in container does not provide a 6 inch gap from the floor to the food surface). Observed food stored in close proximity of the back handwashing sink with water splashes on the box. Food must be removed from the box by 3/27/2020. "Food shall be protected from contamination by storing the food: 1) in a clean dry location; 2) where it is not exposed to splash, dust, or other contamination; and 3) at least 6 inches (15 cm) above the floor."
Corrective Actions: Food was moved away from the handwashing sink and food with water splashes was removed from the box.

15A: .05(6)(a)The ambient air temperature of the reach in prep cooler is greater than 41 degrees Fahrenheit. Individual pizza sauces cold holding greater than 41 degrees Fahrenheit in prep cooler and were discarded (person in charge did not know how long food had been in reach in prep cooler). Facility may not store TCS foods in the reach in prep cooler until the ambient air temperature is 41 degrees Fahrenheit or less. Equipment must maintain in good repair and proper adjustment.

6-1D: .04(6)(i)Observed container of bacon that was made for time control but was not labeled with the start or discard time and melted butter placed on time control from 3/12 that was not discarded. "When using Time Control (TPHC) for a maximum of 4 hours: 1) hot foods shall start at 135°F (57°C) or above and cold foods shall start at 41°F (5°C) or below; 2) the food items shall be labeled with a start and discard time, not to exceed 4 hours; 3) foods not consumed before the discard time shall be discarded; 4) when combining ingredients, the start/discard time for the foods shall reflect the start/discard time of the earliest ingredient placed on time control; 5) written procedures TPHC shall be created in advance and made available upon the Health Authorities request."
Corrective Actions: The bacon and the melted butter were discarded during the inspection.

6-2: .04(6)(g)Observed food that were kept past the seven days date mark. (marinara sauce 3/9, 2 bottles of homemade green sauce 2/26, homemade garlic aioli sauce 3/3, homemade ketchup sauce 3/3, and chicken pot pie 2/25). A food that requires datemarking shall be discarded if it is appropriately marked with a date or day that exceeds 7 days.
Corrective Actions: The sauces and food were discarded.

10D: .04(8)(b)Observe 4 bottles of expired milk 3/15 and 5 containers of expired whip cream 12/17/19. Prepackaged sandwiches, eggs, infant formula, shucked oysters, milk, and time/temperature control safety foods that are labeled as “keep refrigerated” and that are for sale or service to the consumer or used as an ingredient in other foods shall be immediately discarded and shall not be sold, served, or used after the manufacturer’s expiration date or the sell-by date.
Corrective Actions: The milk and the whip cream were discarded.

14B: .05(10)(e) 1,2,4Observed multiple containers that were stored as clean but stacked wet. Clean equipment and utensils shall be stored in a self-draining position that allows air drying and covered or inverted.
Corrective Actions: The containers were rewashed.
