Two restaurants failed this week and two got scores of C. See which they are.

GWINNETT COUNTY, GA — Here are some restaurant and food service inspections conducted in Gwinnett County during the past week. Click on the date for more info.

Each item on an inspection form has a point value ranging from 1 to 9 points, with violations deducting points from a best possible score of 100. Higher points are taken for items with higher risk to cause illness, while repeat violations take even more points.

Letter grades assigned are A for totals of 100 to 90 points, B for 89 to 80, C for 79 to 70, and U for less than 70. Restaurants graded C and lower have their violations listed.

Upon entering facility, no employee took responsibility for being in charge. Person in Charge/manager showed during inspection. There must be a designated Person in Charge with food safety knowledge present at all times.

Observed employee prep produce with bare hands. Produce used for salad. Salad was discarded. Bare hands shall not be used to handle ready-to-eat foods.

Employee drinks (disposable cup and water bottle) stored on prep counter where food was being prepared. Drinks moved to designated employee table. Employee drinks shall be in disposable cup with lid and straw (provided by facility) and stored to prevent contamination of food/food equipment.

Observed raw beef stored on shelf above shelled eggs and seafood in walk-in cooler. Raw beef stored above seafood and vegetables in walk-in freezer. Items rearranged for proper storage. Ready-to-eat foods (cooked foods/produce/sauces) and raw animal foods shall be properly stored/separated to prevent cross-contamination.

Observed employee remove unwashed produce from walk-in cooler. Employee proceeded to cut produce without first washing. Employee grabbed new produce after discarding first and washed in 3-compartment sink. Had employee discard produce. Then had employee wash new produce in correct designated sink. Fruits/vegetables shall be washed in designated sink prior to preparation for human consumption.

Employee phone stored on cutting board where food preparation was occurring during inspection. Phone removed. Employee belongings shall be stored in designated area to prevent contamination of food/food equipment.

Several violations occurred during today's inspection that resulted in an unsatisfactory score due to an overall lack in active managerial control resulting in an unsatisfactory score. The person in charge must be able to demonstrate their food safety knowledge through full compliance of the food code with no violations, be a Certified Food Safety Manager, or be able to provide correct answers to food safey questions as they relate to the food service operation at hand.

No employees working in kitchen had a verifiable method to show they reviewed the employee health policy. Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food.

Observed multiple personal employees drinking bottles without straw instead of used single service beverage cup with secure lid ans straw. Bottles were removed from the kitchen and relocated on the employee assigned area. Employees shall consume food and drink only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas (unless drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee's hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles).

Observed food partially cook such as, salmon, tuna, and halibut without parasite destruction letter. If the fish are frozen by a supplier, a written letter of parasite destruction from the supplier stipulating that the fish supplied are frozen to a temperature and for a time specified under subsection (5)(e) of this Rule may substitute for the records specified under paragraph 1 of this subsection.

Observed ground beef over pork shoulders. Unwashed vegetables above ready-to-eat food. Sliced raw bacon on prep table over vegetables. Foods were reorganized according with the internal cook temperature. Must display, store, prepare, and hold foods so they are protected from cross contamination.

Observed multiple items on reach-in cooler between grills out of temperature compliance. Temperature was over 41oF. Fried onions siting on room temperature. Foods were cooling back. PIC is considering put this food in Time as a Public Health Control. Time/temperature control for safety food shall be maintained at 41°F (5°C) or below.

Observed food storage containers without identification with common name of the food. Food storage containers were labeled with common name of the food. Food storage containers shall to be identify with common name of food.

Observed employees working in the kitchen without beards restraints and hair restraints. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and shallaches longer than one half inch (1/2").

Observed that dish washer machine in the bar area with no sanitizer detected. Hoses were switched. Sanitizer hose and rinse hose were switched and the sanitizer was detected. A warewashing machine that is installed after September 12, 2007 shall be designed and equipped to: automatically dispense detergents and sanitizers and incorporate a visual means to verify that detergents and sanitizers are delivered or a visual or audible alarm to signal if the detergents and sanitizers are not delivered to the respective washing and sanitizing cycles.

During inspection, observed multiple plates of food and food items of employees on the counter in the main kitchen. Person in Charge removed food. Employees shall consume food and drink only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas (unless drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee's hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles).

During inspection, observed beef stored on top of chicken. Observed chicken, chicken spring rolls and duck stored on top of beef in the reach-in freezer. Observed unwashed mushrooms stored on top of ready-to eat foods in the outer walk-in cooler. Observed unwashed produce stored behind cut/ready-to eat peppers. Person in charge rearranged foods accordingly. Raw, unpackaged animal food, such as beef, lamb, pork, poultry, and fish may not be offered for consumer self-service. This paragraph does not apply to: (i) Consumer self-service of ready-to-eat foods at buffets or salad bars that serve foods such as sushi or raw shellfish; (ii) Ready-to-cook individual portions for immediate cooking and consumption on the premises such as consumer-cooked meats or consumer-selected ingredients for Mongolian barbecue; or (iii) Raw, frozen, shell-on shrimp, or lobster.

During inspection, observed facility was using time control on their sushi rice. Facility was using a four rule. Rice had expired according to the time stamp on the sushi rice. Facility did not have any written procedures onsite. Facility discarded rice and was provided an additional copy of time control procedures. "When using Time Control (TPHC) for a maximum of 4 hours: 1) hot foods shall start at 135°F (57°C) or above and cold foods shall start at 41°F (5°C) or below; 2) the food items shall be labeled with a start and discard time, not to exceed 4 hours; 3) foods not consumed before the discard time shall be discarded; 4) when combining ingredients, the start/discard time for the foods shall reflect the start/discard time of the earliest ingredient placed on time control; 5) written procedures TPHC shall be created in advance and made available upon the Health Authorities request."

During inspection, observed multiple containers, squeeze bottles and shakers that were not labeled. Person in Charge labeled foods properly. Food removed from their original packaging and placed in a working container shall be labeled with their common name.

During inspection, observed facility using egg cartons as drain boards to store pots on top of at the cook station. Person in Charge discarded egg cartons and relocated pans to the warewash area to be cleaned. Cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored: 1) in a clean, dry location; 2) where they are not exposed to splash, dust, or other contamination; and 3) at least 6 inches above the floor.

Person-IN-Charge was not knowledgeable of the symtoms and diseases. that employees should report to them as well as what should be reported to the health department. // Went over the symptoms and diseases with PIC and provided handout to PIC. // Person in charge shall explain their responsibility for preventing the transmission of foodborne disease by a food employee who has a disease or medical condition that may cause foodborne disease and can describe the symptoms associated with the diseases that are transmissble through food. They also shall be able to explain how food employees and conditional employees comply with reporting responsibilities and exclusion or restriction of food employees.

During inspection, observed there was no hand washing sink at the bar that stored beverages and glasses to pour beverages in. At least one handwashing sink shall be convenient for employee use in food preparation, food dispensing and warewashing areas as well as in or immediately adjacent to toilet rooms.

Observed the facility missing the TB sanitizer that is documented in their written procedure for cleaning up a vomit or diarrheal incident. // Ensure that TB sanitizer is available on-site. // A food establishment shall have procedures for employees to follow when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. The procedures shall address the specific actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food, and surfaces to vomitus or fecal matter.

Observed cold food items holding an internal temperature above 41°F. // Items were discarded. // Time/temperature control for safety food shall be maintained at 41°F (5°C) or below.

Observed hot holding foods holding an internal temperature below 135°F Food items were discarded. // Time/temperature control for safety food shall be maintained at 135°F (57°C) or above.

Observed multiple cartons of TCS milk in the reach-in cooler that were expired. 2) Observed white American Cheese that had been in use for more than a week that was not date marked. // Items were discarded. // A food that requires datemarking shall be discarded if it exceeds 7 days, not including the time that the product is frozen.

Observed the facility did not have a food thermometer on site. // Please obtain and retain a thermometer in the Kitchen. // Food temperature measuring devices shall be provided and readily accessible for use in ensuring attainment and maintenance of food temperatures.

Observed employee handling food without hair restraints. // Employee put on hair net.// Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and shallaches longer than one half inch (1/2").

Observed utensils being stored in a drawer with old food debris. // Utensils were removed and drawer was cleaned. // Cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored: 1) in a clean, dry location; 2) where they are not exposed to splash, dust, or other contamination; and 3) at least 6 inches above the floor.
