Dist. 123 Offers COVID-19 Vaccine Clinic Saturday at OLHMS
Dist. 123 and Walgreens will offer the Pfizer vaccine to all community members age 12 and up June 19, at Oak Lawn-Hometown Middle School.

OAK LAWN, IL —If you or your kids ages 12 and up still need to get the COVID-19 vaccination, Oak Lawn-Hometown Dist. 123 is hosting a family vaccination clinic on Saturday, June 19. Dist. 123 will be administering the Pfizer vaccine from 8 a.m. to 4 p.m. at Oak Lawn-Hometown Middle School, 5345 99th St. (54th Avenue and 99th Street), Oak Lawn.
The COVID-19 vaccine clinic is open to all community members and is a great way to get everyone vaccinated at the same time. Getting older children vaccinated is the key to schools returning to in-person learning in the fall. Scheduling an appointment is as easy as 1-2-3:
1. Access the Walgreens website and register for an appointment time.
2. Print out the VAR form (Vaccination Administration Record) which has a series of health-related questions. Complete a form for each person and bring the form to your appointment.
3. Bring your insurance card as well – just for recording purposes. The Pfizer vaccination is free.
A parent or guardian must accompany a child under the age of 17. The second dose (required) will be administered at OLHMS on Saturday July 10 from 8 a.m. to 4 p.m. You will be required to complete another VAR form and bring with that day.

People do not need to live in Oak Lawn or Hometown and do not need to be a Walgreens
patient. Walk-ins will be accepted; however, they may experience a longer waiting period.
