Village Of Winnetka: COVID-19 Pfizer Vaccination Opportunity For Ages 16+ On May 5

Anyone age 16 and over can register to receive the Pfizer COVID-19 vaccine at New Trier High School’s Northfield Campus (7 Happ Road, Northfield) on Wednesday, May 5 (1st dose) and Wednesday, May 26 (2nd dose) by registering at the following link (click on the May 5 calendar date and appointments will appear):

Individuals under the age of 18 must be accompanied by a parent or guardian.

The clinic will run from 11 AM to 5 PM. Vaccinations are by appointment only. Vaccination appointments will be made on a first come, first served basis. The May 5 appointment will be the same time of day as your May 26 appointment time, and to register, you must be available on both days. 

Please print and fill out this patient consent form in advance of the clinic and bring a printed copy of your ID and insurance card (on a single sheet of paper) if possible. Doing this in advance will help expedite the check-in process but can also be completed on site if needed. There is no charge for the vaccine and it will be available to those without insurance.  

You must provide your own transportation to and from the site. Masks and social distancing are required at the vaccination site.
