Vaccination Pop-Up Site To Open In Patchogue Next Week

A new pop-up site will be opening in the area next week to distribute vaccines to eligible residents.

PATCHOGUE, NY — A pop-up vaccination will be opening in Patchogue next week, state officials announced on Thursday.

The local site is one of 16 openings at churches, public housing developments, and community centers next week to help administer the COVID-19 to those eligible.

In Patchogue, the Congregational Church of Patchogue on 95 E Main Street will become a vaccination site on Friday, March 19 from 2 p.m. to 6 p.m.

After Friday, the site will then be re-established in three weeks to give out second doses.

A second Long Island pop-up site will also open in Rockville Centre next week.

"We are ramping up our vaccine distribution system to match increases in supply, but a distribution system can't just grow - it has to grow equitably," Gov. Andrew Cuomo said. That's why from the outset we've prioritized the fair and equitable distribution of the vaccine through special efforts like pop-up sites that are being established in the communities that were hit hardest by COVID to help ensure underserved New Yorkers are not left behind in the distribution process."

To make an appointment, call the New York State COVID-19 Vaccination Hotline at 1-833-NYS-4-VAX (1-833-697-4829). Once you have successfully scheduled an appointment, you will receive a confirmation email with a barcode to bring to your appointment.

After making an appointment, you can complete the New York State COVID-19 Vaccine Form. The form should be filled out online and you will receive a submission ID indicating completion. You will need to bring the submission ID to your appointment.

If you cannot submit the form online, it will be available at the vaccination sites.

You must also bring proof of eligibility to your appointment. Depending on your eligibility category, proof can include an employee ID card, a letter from an employer or affiliated organization, a pay stub, a driver's license, passport, or any legal proof of your date of birth and residency.
