City Of Westlake: Covid-19 Vaccination Appointment Locations, Instructions For Phase 1B

The Cuyahoga County Board of Health has released a list of local medical providers that will be hosting vaccination sites for Phase 1B of the state vaccination program, including several federally-qualified health centers, hospitals and pharmacies in Westshore suburbs. These providers will be offering vaccines to individuals 65 years of age and older. 

Below is a list of participating medical providers in and around the City of Westlake. The entire list of Cuyahoga County-based providers can be found on the Cuyahoga County Board of Health’s website HERE.

IMPORTANT: To schedule a vaccine appointment, you must directly contact the provider of your choice from the list of those available. The Cuyahoga County Board of Health has noted that individuals may be able to receive a vaccine more quickly by signing up for a provider from the list than waiting to be notified of a spot opening at one of CCBH’s Covid-19 clinics (www.ccbh.net/vax).  

PHASE 1B SCHEDULE

• The week of Jan. 19: Ohioans 80 years of age and older.
• The week of Jan. 25: Ohioans 75 years of age and older; those with severe congenital or developmental disorders.      
• The week of Feb. 1: Ohioans 70 years of age and older; employees of K-12 schools that wish to remain or return to in-person or hybrid models.
• The week of Feb. 8: Ohioans 65 years of age and older.
