Portland Public Schools Reminds About Coronavirus Vaccination Opportunities For Students Age 12 And Up
Students ages 12-14 must be accompanied by either a parent, a guardian, or someone designated by the parent.

"As we have shared with you previously, the Oregon Health Authority recently announced the approval of Pfizer’s two-dose COVID-19 vaccination for individuals age 12 and up. We are continuing to provide students and their families with vaccination support at the Oregon Convention Center (OCC).
Please visit the PPS Student & Family Vaccination website for information about our supported Wednesday vaccination opportunities and how you can request transportation to and from the OCC. This is the last week to access a first vaccine dose at the OCC. Second doses will be available until they close their doors on June 19.

Students age 15+ can sign up or walk up to the OCC to receive a vaccine. Students ages 12-14 must be accompanied by either a parent, a guardian, or someone designated by the parent. If accompanied by an adult other than a parent, a student must have a parent-signed consent form. Below is the consent form in all PPS-supported languages:
And just a reminder, vaccinations at the OCC and other locations are easy to access throughout the week. You can visit https://getvaccinated.oregon.gov/#/locator to search for vaccination opportunities in your neighborhood or check Multnomah County’s list of vaccination sites."
